CarePlus outreach offers a high quality, community outreach service for adults affected by neurological disorders. Our aim is to provide individuals with the confidence to live independently within the wider community whilst retaining the support of highly skilled therapists and practitioners.

Our flexible support packages can take place anywhere in south-east London. They are available for adults of all ages and are tailored to meet individual needs. We visit individuals who have recently left hospital, residential care or intensive rehabilitation. Rehabilitation may vary from a few hours a week in a number of community settings, to 24-hour support and assistance at home.

We welcome referrals for individuals with cognitive, physical and/or emotional problems related to neurological conditions.
